Wegovy vs. Mounjaro: Which Weight Loss Injection is Right for You?

There is a critical distinction you need to know before comparing these two: Wegovy is the only injection officially approved for weight management. Mounjaro (tirzepatide) is approved for type 2 diabetes — the weight loss it causes is a side effect, not its approved indication. Some doctors prescribe Mounjaro off-label for weight loss, but this is different from formal regulatory approval.

Wegovy is also the only medication in its class with proven cardiovascular protection — reducing the risk of heart attacks and strokes by 20% in the landmark SELECT trial [select_trial].

That said, the weight loss numbers from clinical trials are real: in a direct head-to-head comparison, people taking Mounjaro lost more weight than those taking the standard dose of Wegovy [surmount5_nejm]. The short answer: How they compare

How do they actually work? (The science, simplified)

To understand why Mounjaro causes more weight loss, you have to look at how these drugs interact with your brain and your stomach.

Wegovy is a GLP-1 receptor agonist. It works by copying a naturally occurring hormone in your body called glucagon-like peptide-1 (GLP-1). When you eat, your gut releases GLP-1 to tell your brain you are full. It also slows down how quickly your stomach empties. By injecting a synthetic version that lasts for a full week, Wegovy keeps you feeling full on much smaller portions.

Mounjaro takes this a step further. It is the first approved medication to combine two hormone mimics into a single molecule: GLP-1 and another hormone called GIP (glucose-dependent insulinotropic polypeptide). Scientists sometimes call this a "twincretin."

Adding GIP changes the game. While GLP-1 mainly works on your appetite and stomach, GIP receptors are actually found directly on your fat cells. Activating them helps improve how your body handles insulin and metabolizes fat. Even better, the GIP component seems to tell the brain to calm down some of the nausea typically caused by GLP-1 [knop_dom_2024].

Remember: Mounjaro's primary approval is for type 2 diabetes, not weight management. The weight loss is a documented effect, but Wegovy remains the only injection with a formal weight management indication and proven cardiovascular outcomes data.

Head-to-head weight loss: What the numbers say

For a long time, doctors had to guess how the two drugs compared by looking at separate studies. That changed recently with the publication of the SURMOUNT-5 trial, which tested them directly against each other.

Researchers took 751 adults with obesity and gave half of them the highest tolerated dose of Mounjaro, and the other half the highest tolerated dose of Wegovy. They tracked them for 72 weeks.

The results were clear. People taking Mounjaro lost an average of 20.2% of their body weight, compared to 13.7% for those on Wegovy [surmount5_nejm].

To put that into real numbers: the Mounjaro group lost an average of 22.8 kg (about 50 lbs). The Wegovy group lost an average of 15.0 kg (about 33 lbs).

Mounjaro also helped more people hit massive weight loss milestones. Nearly 20 out of 100 people on Mounjaro lost at least 30% of their body weight. Only about 7 out of 100 people on Wegovy achieved that same level of weight loss [surmount5_nejm].

The new Wegovy HD narrows the gap

Worth noting: the makers of Wegovy recently developed a much higher dose to compete with Mounjaro.

Standard Wegovy maxes out at 2.4 mg per week. The new version, called Wegovy HD, goes up to 7.2 mg. In a recent study called STEP UP, people taking this massive 7.2 mg dose lost about 18.7% to 20.7% of their body weight over 72 weeks [step_up_lancet]. This brings Wegovy's weight loss potential right up to Mounjaro's level. However, this higher-dose version is very new and may take time to become widely available in Kuwait pharmacies.

Side effects and stomach issues

Because Mounjaro produces more weight loss, you might expect it to cause more side effects. Surprisingly, the opposite seems to be true.

Both medications cause similar types of side effects. The most common are nausea, diarrhea, vomiting, constipation, and stomach pain. These usually happen when you first start the medication or when you move up to a higher dose.

But in the direct comparison study, Mounjaro was actually slightly easier on the stomach. Only 2.7% of people taking Mounjaro had to quit the medication because of stomach issues, compared to 5.6% of people taking Wegovy [surmount5_nejm].

Doctors believe this happens because the GIP hormone in Mounjaro helps buffer the nausea caused by the GLP-1 hormone.

That said, everyone's body reacts differently. Some people feel terribly sick on Mounjaro but do perfectly fine on Wegovy, and vice versa. Your doctor will always start you on the lowest possible dose of either medication to let your body adjust.

The heart health advantage: Why Wegovy still matters

If Mounjaro causes more weight loss and has slightly fewer stomach side effects, why would anyone choose Wegovy?

The answer is heart health.

Wegovy has something Mounjaro doesn't have yet: hard proof that it saves lives. In a massive study called the SELECT trial, researchers found that Wegovy reduced the risk of major cardiovascular events—like heart attacks and strokes—by exactly 20% in people who had preexisting heart disease [select_trial].

This is a massive deal in the medical world. Wegovy isn't just a weight loss drug anymore; it is officially recognized as a heart-protective medication.

Mounjaro might also protect the heart—it certainly lowers blood pressure and improves cholesterol—but the long-term studies to prove it are still running. Until those results are published, Wegovy remains the gold standard for patients who have a history of heart problems or are at high risk for a stroke.

Using these medications in Kuwait

Both Wegovy and Mounjaro are approved by the Kuwait Ministry of Health and are available in major pharmacy chains like Boots, Royal Pharmacy, and Ali Abdulwahab Al Mutawa.

Recently, the government mandated price cuts of 30% to 37% for these medications, making them much more accessible. Still, they represent a significant monthly investment, so you will want to handle them correctly.

Here are a few practical things to keep in mind if you live in Kuwait:

Be careful with the summer heat. Both medications are sensitive to temperature. Unused pens must be kept in the refrigerator (between 2°C and 8°C). If you pick up your prescription in July, do not leave the pharmacy bag in your car while you walk around the Avenues. The extreme Kuwait summer heat will destroy the peptides in the medication, rendering it useless. Take it straight home to the fridge.

Navigating Ramadan. If you are taking either medication during Ramadan, you need to plan your meals carefully. Because both drugs slow down how fast your stomach empties, you will feel full very quickly at Iftar. If you try to eat a traditional, large Iftar meal too fast, you will likely experience severe nausea or vomiting. Break your fast slowly with water and a date, wait a bit, and eat small, protein-rich portions.

Staying hydrated. The dry climate in Kuwait combined with these medications can easily lead to dehydration. Both Wegovy and Mounjaro reduce your overall thirst drive. You have to consciously remember to drink water throughout the day, especially if you are spending time outdoors.

Common Questions

Can I switch from Wegovy to Mounjaro? Yes, many people switch if they hit a weight loss plateau or struggle with side effects. However, you cannot just swap them dose-for-dose. Your doctor will usually have you stop Wegovy, wait a short period, and then start Mounjaro at a lower dose to let your body adjust to the new GIP hormone. Always do this under medical supervision.

Do I have to take them forever? The short answer is yes, likely. Obesity is a chronic condition. In the SURMOUNT-4 trial, people who stopped taking Mounjaro regained about 14% of their body weight within a year [surmount4_jama]. These medications only work while they are in your system. If you stop, your natural appetite returns and your stomach empties at its normal, faster rate.

Which one is better if I have diabetes? Mounjaro was originally developed and approved as a diabetes medication (under the same brand name).
